Important Information from Dr. Brady

Bacteria are normal residents of our oral cavity. They are host or normal bacteria and they usually keep harmful bacteria at bay or in balance. Under certain conditions though eg. Poor homecare, they form a biofilm and sticks to the tooth surfaces. The biofilm is like a living carpet and its bond strength to the tooth is enormous. If broken up mechanically or chemically via tooth brushing, flossing, mouth rinses etc., the biofilm will not have a chance to mature and cause disease. When only the gums are affected, the disease is known as gingivitis and this is generally noted by swollen and/or bleeding gums. The bacteria will proliferate if left unchecked and progress to beyond the gums or gingival tissues into the bone and other supporting structures. This is called periodontal disease. Periodontal disease can be categorized as mild, moderate or severe depending on the depth of penetration; that is, how far down the root of the tooth underneath the gums the film has grown. The film hardens with age and is known as tartar. This tartar is what the dentist removes when you go in for a cleaning.

The mouth is the first line of defense against systemic inflammation. It has been linked to diseases such as type II diabetes, rheumatoid arthritis, cardiovascular disease, cancer, obesity and the delivery of preterm, low birth weight babies.

Linked to diabetes by increased level of blood sugars which is food for the bacteria causing them to flourish. The increase in bacteria leads to elevated toxins, therefore more inflammation. Periodontal disease does not cause diabetes but with the slower healing of the diabetic, the condition is more severe and aggressive.

Linked to rheumatoid arthritis via the decrease in ability to floss and therefore remove plaque from between the teeth,(approx 40%). This causes bone loss and inflammation of the gums.

Linked to cardiovascular disease via an increase in levels of C reactive protein (marker for inflammation). Patients with gum disease i.e. periodontitis have an increased level of atherosclerotic plaques and leukotrienes (lipid or fat derived inflammatory mediators). Also, germs form the infected gums have been found living in the atherosclerotic plaques thus linking the diseases together.

The link with cancer is very new and thus controversial. Studies have found links with lung, kidney and pancreatic cancers as well as cancers of the blood. Researchers think “periodontal disease may be a marker of a susceptible immune system or might directly affect cancer risk”

Patients who have liver disease have also been associated t with periodontal disease.

Obesity has been linked to periodontal disease by the elevation of C reactive protein which can then cause the inflammatory cytokines and hormones secreted by adipose tissue to be stimulated. C-reactive protein causes fat cells called adipocytes to store more fat and burn less energy.

It has also been noted that pregnant women with periodontal disease have a higher incidence of delivering babies preterm and the babies born to these mothers have a lower than normal birth weight even if delivered at term.
